samenvatting: GnRH analogues are the standard therapy but expensive therapy for the suppression of pubertal changes in transsexual individuals. In the 1960s, medroxyprogesterone was found to be efficacious in treating central precocious puberty by inhibiting the secretion of gonadotropins and/or interfering with gonadal steroid synthesis. To avoid the adverse effects and prohibitive cost of gonadotropin releasing hormone analogues, this study utilized medroxyprogesterone as an alternative treatment for puberty sex hormone suppression. The goal of this retrospective chart review is to determine the efficacy and safety of medroxyprogesterone. Sixteen subjects with Gender Identity Disorder less than 19 years were offered medroxyprogesterone to suppress puberty sex steroids. Seven male-to-female individuals were treated with the oral form of medroxyprogesterone. Six female-to-male individuals used depot medroxyprogesterone acetate; one used oral; and two refused. One decided to change back to being female. None of the patients discontinued therapy because of unwanted side-effects. In conclusion, medroxyprogesterone is an effective, safe, and affordable option for the suppression of pubertal hormones in teens desiring gender change. It is also an excellent option for those who have needle phobia. Response to treatment and compliance were favorable.

Arresting Ashley X : Trans Youth, Puberty Blockers and the Question of Whether Time is on Your Side  / Jake Pyne.

samenvatting: In 2004, a young Seattle girl with significant disabilities known only by the pseudonym Ashley X, underwent a series of medical procedures without her consent. At the request of her parents, Ashley received a mastectomy, a hysterectomy and hormonal treatment, designed to arrest her development in a child-like state. In the eyes of her doctors, her family, and their lawyers, it was urgent that Ashley's body be aligned with her purported cognitive age. The temporal and ethical arguments used to justify her 'Treatment' turned Ashley's body into a site of international debate over disability and human rights, consent and medical science, eugenics and human engineering. Yet the similarities and differences between Ashley's non-consensual pubertal arrest versus that actively sought by trans youth are rarely mentioned. This paper uses the clinical and media discourses at work in both the 'Ashley Treatment' and the treatment sought by trans youth to think through this moment in which some bodies are treated to greater forms of autonomy while other bodies have none. I argue that the discourses used to secure transition for some trans youth ought to be weighed against the implications for others, in particular for trans youth with disabilities. While the availability of puberty suppression for trans youth can be narrated as a sign that things are getting better, the literatures of queer temporality and critical disability studies help to consider that it may also augur something else - a widening gap between those invested with the ability to stop time versus those who are stopped in time.

samenvatting: To describe the use of hormonal contraceptives for menstrual management and/or pregnancy prevention in a clinic-based series of transgender adolescents and young adults who were assigned female at birth (transmasculine identity). Methods: We performed a chart review of post-menarchal transgender assigned-female-at-birth (AFAB) patients, age 10-25 years, seen at CCHMC Transgender Health Clinic for at least 2 visits between July 1, 2013 and September 17, 2016, and who were not on a puberty suppression method. We collected data including choice of hormonal contraceptive and indication (menstrual suppression, pregnancy prevention, or both), duration of use, initiation of sexual activity, reported sexual partners, and use of gender-affirming hormone therapy (i.e., testosterone). We present simple descriptive statistics. Results: A total of 231 patients met inclusion criteria, with ages from 11 to 25 years. Of those, 135 (59%) were using a hormonal contraceptive method. Most patients (67%) used hormonal contraception for the indication of menstrual suppression. Most commonly used method was depot medroxyprogesterone (DMPA) (49 patients), followed by combined oral contraceptives (COC) and norethindrone (progestin-only pill, POP) (34 patients each). Thirteen patients used 52 mg levonorgestrel IUD (LNG-IUD). Of the total sample (n = 231), 82 (36%) reported sexual activity, 35 of whom (43% of sexually active patients) reported sexual intercourse with assigned-male-at-birth (AMAB) partners and/or penile-vaginal intercourse. Among 35 patients at risk for pregnancy, only 21 (60%) were using hormonal contraception. Over half (54%) of sexually active patients taking testosterone discontinued their hormonal contraceptive method once they stopped having menses. Discussion: Within a sample of transgender AFAB adolescents, half of whom were taking testosterone, a variety of contraceptives were used, including depot medroxyprogesterone, combined oral contraceptives, and levonorgestrel IUD. Among those taking testosterone, many patients discontinued contraception once they stopped having menses.

samenvatting: The past 15 years have seen the growth of puberty suppression as the prevailing approach to supporting gender non-conforming children and youth. Puberty suppression is considered to provide time for weighing up the pros and cons of medical transition. Research based on binary understandings of gender has demonstrated that a carefully selected group of gender non-conforming youth benefit from physical treatment and gender transition, but the research that details how psychologists can best support young people during this time is limited. This is the gap addressed by the current research. The purpose of the present study is to explore the meaning-making framework within which some clinical psychologists and gender non-conforming youth approach discussions of puberty suppression. Five semi-structured interviews were conducted with clinical psychologists working with gender non-conforming youth. The data were analysed using thematic analysis. The results indicate that there is pressure on gender non-conforming youth, often coming from families, friends and mass media, to buy into heteronormative and binary discourses regarding gender and what constitutes a good life. The results also indicate that the participants deploy affirmative and exploratory therapeutic strategies in their work, in order to enable gender non-conforming youth to make informed decisions regarding puberty suppression. Participants emphasized the importance of therapeutic approaches that explore non-binary gender discourses, alongside the use of puberty suppression and other medical interventions that enable clients to fit more with gender norms. The therapeutic balance between affirmation and exploration may shed light on how both research within the binary tradition and critics of binary assumptions are in danger of oversimplifying the process of gender identity development. This research highlights the importance of understanding the complex negotiation of gender discourses that are in tension with one another.

samenvatting: Danser Louis Alves kreeg op 18-jarige leeftijd groen licht van de genderpoli om met vrouwelijke hormonen te starten. Kort daarna ontving hij een scholarship om twee opleidingen in Amerika te volgen. Hij vertrok naar New York en toen hij met de diploma's op zak weer terug in Nederland was, besloot hij niet in transitie te gaan. Als gevestigde danser en docent blikt hij terug op dit proces en vertelt over de invloed die de omgeving heeft gehad op zijn zoektocht naar zichzelf.

samenvatting: In this editorial we unpack and analyze the claim that prescribing puberty delaying medications is experimental and we show that provision of puberty delaying medications to adolescents with gender dysphoria is not experimental, or at least not any more experimental than standard pediatric practice when there are no licensed treatment options for a pediatric patient population. We will analyze three issues in particular: 1) Does the fact that the drugs used for inducing and maintaining puberty delay are prescribed 'off label' make the use experimental?; 2) does the fact that the drugs do not have market authorization for puberty delay in gender diverse children make the use experimental?; and 3) does the fact that there are no randomized controlled trials of puberty delay in gender diverse children make the use experimental?

This is a claim for judicial review of the practice of the defendant, the Tavistock and Portman NHS Foundation Trust, through its Gender Identity Development Service (GIDS) and the first and second Interveners (the Trusts) of prescribing pubertysuppressing drugs to persons under the age of 18 who experience gender dysphoria. The claimants' case is that children and young persons under 18 are not competent to give consent to the administration of puberty blocking drugs. Further, they contend that the information given to those under 18 by the defendant is misleading and insufficient to ensure such children or young persons are able to give informed consent. They further contend that the absence of procedural safeguards, and the inadequacy of the information provided, results in an infringement of the rights of such children and young persons under Article 8 of the European Convention for the Protection of Human ights and Fundamental Freedoms (the Convention).
