samenvatting: Women who have sex with women and men (WSWM) are less likely to use effective contraception and more likely to experience unintended pregnancies than their exclusively heterosexual peers. The authors examined contraception use among a community sample of 80 WSWM (defined as genital contact with at least one cisgender man and woman during the past 12 months) of reproductive age living in the Midwest. Authors determined if contraceptive use differed by sociodemographic characteristics, or sexuality and relationship characteristics, and used a modified timeline follow-back calendar to explore contraception use during most recent penile-vaginal intercourse (PVI). Overall, use of highly effective contraception was low (38.8%) with combined hormonal contraceptive pills reported most frequently (54.8%). Of participants who engaged in PVI during their most recent sexual event with a male, nearly one half (46.1%) reported using at least one form of contraception. WSWM with a primary sexual attraction to men were more likely to use contraception (p < .01), as were those who recently engaged in PVI (p < .05). Self-identified lesbian participants were less likely than all others to use contraception (p <. 05). WSWM not planning to engage in sex with men may not use contraceptives consistently or at all, suggesting an area where inclusive contraception counseling can improve the reproductive health of women who have sex with women and men.

samenvatting: To describe the use of hormonal contraceptives for menstrual management and/or pregnancy prevention in a clinic-based series of transgender adolescents and young adults who were assigned female at birth (transmasculine identity). Methods: We performed a chart review of post-menarchal transgender assigned-female-at-birth (AFAB) patients, age 10-25 years, seen at CCHMC Transgender Health Clinic for at least 2 visits between July 1, 2013 and September 17, 2016, and who were not on a puberty suppression method. We collected data including choice of hormonal contraceptive and indication (menstrual suppression, pregnancy prevention, or both), duration of use, initiation of sexual activity, reported sexual partners, and use of gender-affirming hormone therapy (i.e., testosterone). We present simple descriptive statistics. Results: A total of 231 patients met inclusion criteria, with ages from 11 to 25 years. Of those, 135 (59%) were using a hormonal contraceptive method. Most patients (67%) used hormonal contraception for the indication of menstrual suppression. Most commonly used method was depot medroxyprogesterone (DMPA) (49 patients), followed by combined oral contraceptives (COC) and norethindrone (progestin-only pill, POP) (34 patients each). Thirteen patients used 52 mg levonorgestrel IUD (LNG-IUD). Of the total sample (n = 231), 82 (36%) reported sexual activity, 35 of whom (43% of sexually active patients) reported sexual intercourse with assigned-male-at-birth (AMAB) partners and/or penile-vaginal intercourse. Among 35 patients at risk for pregnancy, only 21 (60%) were using hormonal contraception. Over half (54%) of sexually active patients taking testosterone discontinued their hormonal contraceptive method once they stopped having menses. Discussion: Within a sample of transgender AFAB adolescents, half of whom were taking testosterone, a variety of contraceptives were used, including depot medroxyprogesterone, combined oral contraceptives, and levonorgestrel IUD. Among those taking testosterone, many patients discontinued contraception once they stopped having menses.

samenvatting: Trans masculine people are more likely than cisgender peers to have a teen or unintended pregnancy, though little is known about the origins of these disparities. This study aimed to describe teen and unintended pregnancy experiences among trans masculine people in order to elucidate risk factors and pregnancy-related needs. As a part of the United States-based SexuaL Orientation, Gender Identity, and Pregnancy Experiences (SLOPE) study, in-depth, semi-structured interviews were conducted between March 2017 and August 2018 with 10 trans masculine people, ages 20?59 years, who experienced a teen or unintended pregnancy. Audio-recorded interviews were professionally transcribed, then analyzed using immersion/crystallization and thematic analysis approaches. The themes were contextualized using sociodemographic survey data. The four themes that were developed from participants' narratives highlighted: 1) how trans masculine people navigated having a pregnant body (e.g., heightened gender dysphoria due to being pregnant); 2) the importance of the cultural environment in shaping experiences as a trans masculine pregnant person (e.g., pregnancy and gender-related job discrimination); 3) the development of the pregnancy over time (e.g., decision-making processes); and 4) how pregnancy (and gender identity) affected relationships with other people (e.g., adverse family of origin experiences). Discussion : This study identified a number of risk factors for teen and unintended pregnancies among trans masculine people including physical and sexual abuse as well as ineffective use of contraception. This research also identifies unique needs of this population, including: relieving gender dysphoria, combating discrimination, and ensuring people feel visible and welcome, particularly in reproductive healthcare spaces. Public health practitioners, healthcare providers, and support networks (e.g., chosen family) can be key sources of support. Attention to risk factors, unique needs, and sources of support will improve reproductive healthcare and pregnancy experiences for trans masculine people.

samenvatting: Study on Carlile's book originally published as 'Every Woman's Book, or, What is love?' in 1826, including the original text. It scored a double first: as a progressive sex manual and as the first book in English to specify methods of contraception. Carlile, a leading radical of the early nineteenth century, was notorious for his blend of republicanism, atheism and libertarianism. His book created a philosophy of sex which was found irresponsible and obscene at the time but today appears sensible and progressive. It proposed that sex was a right of adulthood rather then a marital privilege; that sexual satisfaction was necessary to a person's health and happiness; that sexual intercourse was a virtue and therefore a source of morality; and that men and women both had the same sexual needs and were therefore naturally equal. Michael Bush provides a lucid and entertaining introduction to Carlile, a man opposed to alcohol and meat-eating but committed to the 'health-giving' properties of raw mercury, and who served ten years in prison for his beliefs, during which he organised an extensive following of fellow republicans and atheists. Bush's careful analysis of the personal and intellectual origins of Carlile's work and its impact do full justice to what is both a fascinating historical document and a key influence on society's acceptance of a liberal, sensualist view of sex.

samenvatting: Black women who have sex with women and men (BWSWM) are at risk for unplanned pregnancies, human immunodeficiency virus (HIV), and other sexually transmitted infections (STIs). However, there is a paucity of studies assessing psychosocial factors related to sexual risk in this population. This study investigated the relationship among condom use; contraceptive self-efficacy; lesbian, gay, and bisexual (LGB) identity; sexuality-religious conflict; parent support for participants' attraction to women; and LGB community support within a sample of 162 participants who responded to an online survey. Participants were cisgender women (n = 152) and genderqueer or gender-nonbinary participants who were assigned female at birth (n = 10) aged 18 to 35. Hierarchical linear regression analyses indicated that LGB identity, low sexuality-religious conflict, and greater parent support predicted contraceptive self-efficacy and, in turn, greater contraceptive self-efficacy mediated the relationship between these factors and increased condom use. Implications for work with this population in research and clinical contexts are considered.

samenvatting: Hormonal treatments and surgical interventions practiced with the aim to affirm gender identity in transgender and gender diverse patients may impact their future reproductive ability, family building, and family planning options. Whereas it is recommended by international guidelines to discuss the potential risks of infertility and to present fertility preservation (FP) options to transgender individuals and their families prior to initiating any of these treatments, many barriers still remain. Further, transgender and gender diverse individuals often experience barriers to accessing contraception, abortion, pre-conception care, and comprehensive perinatal care.
