Satisfaction of MtF Transsexuals with Operative Voice Therapy : A Questionnaire-based Preliminary Study  / Kerstin Neumann [et. al.].

samenvatting: After having undergone successful sexual reassignment surgery male-to-female (MtF) transsexuals are very often negatively affected by their unchanged masculine voice, which contrasts with their feminine appearance and this interferes considerably with their social integration as women. Since hormonal treatment or logopedic therapy alone will not achieve a permanent increase in voice pitch, the development of a phonosurgical technique to achieve a permanent elevation of the voice pitch is of great importance. Since 1993, 67 MtF transsexuals have undergone cricothyroidopexy at the department of otorhinolaryngology, in the university hospital in Halle, according to Berghaus and Neumann (1996). Increased tension of the vocal cords results in a permanent elevation of the fundamental frequency range by five to six semitones on average. While pre-operatively none of the patients had a feminine speaking voice pitch, after the surgery, about 30 percent attained a voice pitch within the feminine range, and 38 percent attained at least a neutral voice pitch. In the long-term, mostly combined with further voice training, the results proved to be of a permanent character, and in 45 percent of the cases, a further increase was observed. The operation technique in question has proved to be generally successful with a minimum of risk factors. The effect that voice therapy can have on the social integration of the persons in question and to what extent they were satisfied with the results of voice treatment was assessed by means of a questionnaire, which was developed with the Institute of Psychology at the Martin-Luther University of Halle-Wittenberg. Twenty-eight questionnaires were evaluated, and in most cases, the feminisation of the voice had improved and facilitated better social integration was achieved by the MtF transsexuals who had undergone treatment.

samenvatting: The objective of this study was to examine the voices of male-to-female (MtF) transgender veterans and biological females that can result in identification or misidentification of biological gender. Twenty-one MtF transgender veterans and 9 cis-gender females were enrolled. The interaction of speaking fundamental frequency (SFo) and formant (resonatory) frequencies in gender discrimination was investigated. The results indicated that an average SFo above 180 Hz and maintaining a speaking pitch range of approximately 140 to 300 Hz appear to be the most powerful acoustic features or markers in the perception of a female voice in a biological male (M. L. Brown & Rounsley, 1996). An SFo of approximately 170 Hz appears to be the lower limit that would result in a biological male being perceived as having a female voice by most listeners. A slight elevation in the second (F2) and third (F3) formants was noted but does not appear to have a significant influence in the perception of a female voice in biological males. Female voices appear to be perceived as male by most listeners if average SFo is at or below 165 Hz, the low SFo is below 130 Hz, and a low F3 is exhibited. No evidence was found that jitter (frequency perturbation) and shimmer (amplitude perturbation) affect the perception of a female or male voice in a biological male. The results support previous research that elevated pitch is the strongest acoustic marker in the perception of a female voice in biological males. samenvatting: This study evaluated (a) educational background, experience, and confidence among speech-language pathologists in providing services to transgender individuals and (b) awareness of transgender speech services among the lesbian, gay, bisexual, and transgender population. Two anonymous surveys were sent to practicing speech-language pathologists and transgender participants in Illinois; 228 speech-language pathologists and 88 transgender participants responded. A majority of speech-language pathologists indicated that providing services to transgendered individuals was within their scope of practice but felt improperly prepared to provide assessment and treatment. Almost half of the transgender individuals were unaware of speech and language services to address their needs during transition. samenvatting: In the seventh version of their Standards of Care, WPATH recognizes that, as each person is unique, so is the person's gender identity. The goal of speech-language therapists/pathologists is to help transgender people develop voice and communication that reflects their unique sense of gender. When outer expression is congruent with an inner sense of self, transgender people may find increased comfort, confidence, and improved function in everyday life. Transgender voice and communication is a relatively new area of practice within speech-language pathology/therapy and this document is intended to support clinicians and researchers working in this field. It begins with a review of the evidence-based literature in transgender voice and communication. The paper then discusses these clinical topics: trans-specific voice-and-communication assessment, voice feminization protocols and voice feminizing surgeries. There is also a section on speech and voice masculinization - an area that has received little previous attention. As minimal standards have yet to be established in this field the paper concludes with recommendations for good clinical care. samenvatting: The recently developed Transsexual Voice Questionnaire (TVQMtF) is a self-report tool that assesses the perceptions of male-to-female transsexual women (MtF women) regarding their voices and the impact of voice on their everyday lives. Initial psychometric evaluation has demonstrated strong reliability of the TVQMtF, however, there has been relatively little investigation of its validity. There is evidence in the literature to support the notion that MtF women who have undergone gender reassignment surgery (GRS) have different perceptions of their voice than MtF women who have not undergone GRS. This information was used to examine the validity of the TVQMtF using a known-groups analysis. Method: The TVQMtF scores of 26 MtF women who had undergone GRS were compared with the scores of 27 MtF women who had not undergone GRS. A one-way analysis of variance (ANOVA) was used to compare the total TVQMtF scores. Mann Whitney U tests were used for comparison across individual items. Results: Individuals who had undergone GRS perceived that they experienced fewer voice-related difficulties and impacts on their lives than individuals who had not undergone GRS. This difference was significant (F(1, 51) = 8.87, p = .004). MtF women who had undergone GRS reported less impact of voice on their gender identity, however, perceived impact of vocal-functioning and voice-related activity and participation limitations did not differ between groups. samenvatting: Speech and language therapists (SLTs) deliver voice and communication therapy for trans and gender diverse people to facilitate authentic vocal and communicative expression. Davies, Papp, and Antoni (2015) have provided a comprehensive review of the literature, with recommendations for good clinical care. Several areas highlighted as gaps in the research were identified by the current authors as ones in which evidence is expanding. Aims: To demonstrate 1) an expansion of the evidence base in particular innovations in voice group therapy for trans women and trans men; 2) the importance of delivering voice and communication therapy as part of a complete approach to trans and gender diverse health care; and (3) developments in training and competency in the UK. Method: Data were drawn from three small-scale projects, two surveys, and one audit. Measurements: Data from survey and audits and pre- and post-group acoustic measures of and client self-perceptual measures, including the Transsexual Voice Questionnaire (TVQMtF); a client-generated set of questions for trans men; and the migration of vocal identity map, adapted from Narrative Therapy practices. Results: Positive outcomes for both pitch measures and client perception were recorded. Audit and survey data provided evidence of developments in training and competency in the UK. Conclusion: Evidence supports group therapy as a successful approach for trans individuals across a number of parameters of voice and self-perception. Voice group protocols for both trans men and trans women should take account of the social context within which to explore relational presence and authentic voice. SLTs within gender identity clinics provide voice and communication as part of a broader pathway of care, alongside sharing professional knowledge and skills. Current UK developments are documented as indicators of positive responses to the growth in the number of SLTs seeking to develop specialist skills within this field.

samenvatting: Testosterone therapy is the predominant treatment for voice masculinization in transgender patients. Although lowering of voice fundamental frequency (f0) occurs with testosterone therapy, evidence suggests voice and gender identity may not fully align?i.e., voice-gender congruence may not be achieved?with its use. Aim: This meta-analytic review evaluates the effectiveness of testosterone therapy to masculinize voice in transgender patients. Methods: Multiple electronic databases were queried (inclusion dates: from database inception up to October 27, 2017) to identify original research on voice masculinization using testosterone therapy. Nineteen articles were included in this meta-analytic review, which followed PRISMA guidelines. In addition to qualitative analyses, random effects proportion meta-analyses were performed on data related to f0, voice-gender congruence, voice problems, and satisfaction with voice. Results: A meta-analysis on f0 data showed after 1 year of testosterone therapy a combined estimate of 21% of participants (95% confidence interval [CI]: 5%-43%; I2: 59.9%) did not achieve cisgender male normative frequencies (f0 ? 131 Hz). Meta-analyses on incomplete voice-gender congruence and voice problems indicated combined estimates of 21% (95% CI: 10%-34%; I2: 0.0%) and 46% (95% CI: 14%-79%; I2: 90.2%), respectively. Regarding incomplete satisfaction with voice, a meta-analysis showed a combined estimate of 16% (95% CI: 7%-28%; I2: 0.0%). Discussion: We found that not all transgender patients using testosterone therapy to masculinize voice should expect f0 lowering to cisgender male normative frequencies after 1 year. The vocal transition may involve voice problems for many patients, and some might not achieve voice-gender congruence without additional, voice-specific intervention. Given these findings, a voice evaluation should occur prior to initiating testosterone therapy and involve counseling on expectations for voice. Transgender patients who pursue voice masculinization may need management from laryngology and speech and language therapy to improve voice-gender congruence, mitigate voice problems, and increase satisfaction with voice.

samenvatting: One of the most influential factors that affect the quality of life of transgender individuals is whether they can be perceived by others to "pass" in their felt gender. Voice and communication style are two important identifying dimensions of gender and many transgender individuals wish to acquire a voice that matches their gender. Evidence shows that few transgender individuals access voice therapy, and that this is caused by their concerns about stigmatization or negative past experiences within healthcare services. In order to address the negative experiences faced by transgender populations we need a better understanding of healthcare services' current levels of knowledge and LGBT awareness. Some studies of Speech-Language Therapists' (SLTs') experience and confidence working with transgender individuals have recently been undertaken in the United States (US). However, little research has been carried out in Asia. Aims: To investigate Taiwanese SLTs' knowledge, attitudes and experiences of providing transgender individuals with relevant therapy. Method: A cross-sectional self-administered web-based survey hosted on the Qualtrics platform was delivered to 140 Taiwanese SLTs. Results: Taiwanese SLTs were, (i) more familiar with the terminology used to address "lesbian, gay, and bisexual groups" than with "transgender" terminology, (ii) generally positive in their attitudes toward transgender individuals, and (iii) comfortable about providing clinical services to transgender clients. However, the majority of participants did not feel that they were sufficiently skilled in working with transgender individuals, even though most believed that providing them with voice and communication services fell within the SLT scope of practice. Conclusion: It is important for clinicians to both be skilled in transgender voice and communication therapy and to be culturally competent when providing services to transgender individuals. This study recommends that cultural competence relating to gender and sexual minority groups should be addressed in SLTs' university education as well as in their continuing educational programs.
